Because even though we are more accepting now and doesn't same-sex relations like the total "out-group" that it used to, our culture still tends to give it specific acceptance. That is to say, "it's okay (the 'in-group') for homsexual people, but not okay for me (still treating it as an 'out-group')." And so even though they may be okay with homosexual relationships as a concept, some heterosexual people still have difficulty being exposed to any related imagery.

Essentially, it's a lingering part of culture from a time when it wasn't okay.
