For me, the drinking was implied. I usually don't order appetizers unless I'm going somewhere with friends where drinking is pretty much assumed. Or even a bar where you're pretty much eating nothing BUT pub food (and my go-to appetizers there used to be mozzarella sticks and chicken fingers).

I rarely get onion rings anywhere, though, because it seems like very few places ever really do them right.

No clue - as far as I can remember, any time I order them I mostly get the same size, so I have no idea if the ones I'm thinking of are what you'd classify as thicker or thinner.

That's sounds more a question of whether or not they've cooked them well or not. Mozarella can be a VERY chewy cheese if you don't heat it enough. It's basically the difference between a red-hot pizza topping and a stick of string cheese.

Though talking about chewy mozzarella cheese, I am now reminded of the time I threw up after a night of drinking, where I'd eaten double cheese pizza the night before, and my vomit basically formed a solid rope of cheese from my throat out of my mouth. I literally had to reach into my mouth and sort of tear it off to throw most of it away and swallow down the rest, because it was sort of stuck in there.

It was unpleasant enough that I've never ordered double cheese as a topping since. Though not so unpleasant that it led me to stop eating pizza (or drinking) entirely.
