There's a few fixed cities every few milestones, but they are VERY VERY far apart. I think the areas between these fixed cities are procedurally generated, but I also think the game was multiplayer so that doesn't seem right... I can't remember probably not, but just so I have something to say, what you describe sounds partially like Zangarmarsh in WoW. if what you're describing is a dream or a blending of memories it could be related, but probably not. --- ...
