Reading the full article, though, it makes sense. They're not waiting for a literal response to give the go ahead to change the diaper, they're wanting children to be raised from the start knowing that how they feel should be respected and to speak up when/if the time *does* come. There's a lot of child sexual abuse towards children who don't say anything, whether it's from fear of being in trouble, or they view it as normal because it's from someone they know so when they don't like it, they don't say anything.
