In this country, insurance companies and the government (via medicare/aid) negotiate prices, which is exactly why those prices start grossly inflated, so that the price can be "negotiated" down to close to what would have been asked in the first place.
If I ask you for a dollar, and you insist that you're only paying 33 cents, you're getting charged three dollars next time.

Also we're essentially subsidizing the "negotiated" rates elsewhere.
