Usually that means vast expanses of procedurally generated blandness. you say that like most open world games worlds aren't just mostly randomly generated if you think everything is crafted by hand, then jeez There's a big difference between games that start by generating a map and then tweak it and games that just let the algorithm loose and call it a day. The amount and quality of hand crafted stuff tends to make or break an open world game and any small scale developer trying to make a huge world is all but guaranteed to skimp on it. --- And with that...pow!
