I generally start with normal. Harder difficulties, I tend to leave for subsequent playthroughs, unless I'm given reason to believe that's where to start (in FE13, for example, I knew that Hard was pretty comparable to the Normal of prior FE games, so I started there).

Also this. If I like the game (or I want the trophies), I'll go back at a harder difficulty. The vast majority of the time the added difficulty doesn't really add much anyway, just buffing enemies' damage or health which is lazy. In some cases it even does to illogically, like requiring 2 headshots to kill an unhelmeted opponent instead of 1.
