Having watched a ton of Plainly Difficults industrial accident videos, Im going to guess it was a mixture of shoddy maintenance combined with good old fashioned complacency that likely caused this, as thats been the bane of many other industrial plants over the years.

Hopefully everybody made it out safely, though given how bad of an explosion it was, theres likely to be a lot of casualties among the personnel and the surrounding civilian population.
