All secret exits could be accessed the first time you access a level. No levels have more than one secret exit.

In addition to red dots, some Ghost Houses also have secret exits too.

Well...there's some that are a little difficult to get without some setup before hand (switch blocks, Yoshi, etc.).

Good point about the Ghost Houses. Those are easy to miss.

I just played through World recently and I think I got like 90ish of the exits, despite doing my best to get them all along the way. There were a few at the very end (in the Bowser area) that I just didn't feel like getting because I was trying to finish the game in one day. But I definitely missed a few even accounting for that.
