I was a shift lead for a competitor for several years back in college: it's not difficult. Most register systems allow you to annotate alterations easily enough, and the callout to the assembler is simple enough.

But as I already said, I get that the average fast food worker short-circuits. That's why I said it's best for her to just ask for the sausage to be kept 'on the side'. If they can't accommodate that then they're so stupid that calling corporate will result in free gift cards for the complaining party. :P

E.g. we had a regular to liked to share a hamburger patty with his dog, so he literally asked for the patty to be kept on the side. Not a difficult request by any means.
