The insurance was billed around $650 for the debridement part of the check-up and the rest was the office charge. My out of pocket is $263. That's more expensive than before I had health insurance, when I paid with just cash.

Uh, debridement is a minor form of surgery. That's not normally part of a checkup. What is the doctor doing that could be considered "debridement"? That could be the reason for the high cost, or it could be fraud. If it's a normal checkup you shouldn't be getting charged for debridement.
