Honestly the whole set up for Anakin going dark was kinda stupid. It was a self fulfilling that led to Amadala croaking, although perhaps Palapatine planted those images in his head?Anyway there where tons of ways for him to prevent her death, and if he had a real Master (Obi really was dropping the ball by that point), he probably would have seen that. Obi basically thought Ani was ready to be on the council and be a full Jedi etc (maybe because he had saved his bacon a bunch of times), but he was VERY wrong.
