Your thumb should pretty much always be at the angle you're using for the A button. You don't have to completely reorient your hand for the usage of the A button. If it's at the same angle you're using for the A button, you can keep that same angle for the B button. Simple, easy switching between the two. Not to mention, if you're so keen on having Y & B being your primary buttons, like if you're playing SMW, then your thumb should still be at the angle you're using for the A button, so the tip of your thumb can be on Y, and the base of it on B. I mean, the layout for the SNES's button layout is designed for your thumb to always, comfortably be at an angle like you're using for the A button.
