Basically the catcher stands up with his arm way out of the strike zone on the otherside of the hitter like so (a very common sight as a giants fan in the early 2000s when Barry Bonds was hitting)

It signals that they don't want to pitch to him, and the pitcher basically lobs 4 unhittable pitches.

That's the literal definition of it, and that is what counts towards Bonds' record breaking 2004 total. He's also one of 6 players to be intentionally walked with the bases loaded (which means that this forced a run home).

They just changed the rule this year so that there's no actually pitches involved anymore (basically the batter is automatically awarded the base) to speed up the game and because some shitty pitchers struggle with the throw and miss the target
