Butter on both sides AND in the pan, for cheese Kraft singles is alright (TC) I prefer Velveeta or if you want to get fancy Cooper Sharp. The bread I use is Italian since it makes for a wide looking sandwich...the ratio I leave a bit of room open so it can melt properly without sticking to the pan much.
Velveeta has the wrong consistency for grilled cheese IMO it's meant for things like Mac & cheese or making cheese dips. Kraft singles on the other hand is basically formulated specifically to make quick & dirty grilled cheese.
