I think the whole thing was to create drama. They've been really inconsistent with injuries/ejections during challenges, as well as the consequences of medivacs during challenges, especially the past few years:

Samoa:
-Ben gets ejected from a challenge for roughhousing with Russell, Foa Foa has to compete with one less person. Additionally, and injured Mike has to continue competing. Mike is medivac'd but the losing Foa Foa tribe still has to vote out another member.
-Russell gets medivac'd during a challenge, Jeff calls the challenge off and declares it a draw. On a night where both tribes were supposed to vote someone out, now neither has to with Russell gone.

Heroes vs. Villains:
-James gets injured during a challenge, he gets pulled and the Heroes have to compete with one less person, despite Colby sitting on the sides having sat the challenge out, he can't be brought in.

One World:
-Kourtney gets injured, Jeff gives the guys the option to take their default victory or wait and see if Kourtney comes back. They take the default victory, but they would've been given it anyway. The "losing" Salani tribe does not have to vote out another member with Kourtney gone.
