It's definitely not appropriate to just assume people who use pain medications are addicts. I don't agree with all of the bashing, and I hate that addicts and prescribing practices from 10 yrs ago spoiled it for people like TC. I'm with you on that. Nobody should be assuming that someone is an addict especially if they know their pain regimen, and pain is a real and debilitating thing. It sucks, but you do have to be cautious and have your own license in mind when it comes to controlled substances. I'm happy this was at least ironed out for the TC at the end of the day.

One complicating issue is that this patient was a resident who does not have her own DEA number and is acting under her attending. I usually don't deal with chronic pain patients (and didn't in my residency), but during residency I could not write or e-scribe opioids for outpatient post-op pain--it had to be done through an attending's triplicate which they had to personally write. I know benzos are C-IV scheduled drugs which makes them less controlled than opioids, but I can't remember if they needed a triplicate for outpatient management. There's a chance that it's more on the attending than the resident, since the attending may have refused to fill meds with abuse potential for initial visit patients. A lot of people who don't deal with controlled meds avoid prescribing them because of all the litigation around them. I will avoid filling chronic pain meds since that's not my wheelhouse.

I've had the monthly patient complain about me and tell the nurses about how they've never been told X, Y, or Z in contrast to the management plan, or how their prescriptions were never filled, or why am I not scheduled for surgery. And then we go to the medical record and it's all been documented over months of counseling. People will swear up and down that they never used X treatment, and they never got Y imaging, or have never done Z procedure and I'm lying. And it's all documented with reports and pharmacy fills in their chart. Patients are NOT malicious, some of them just horribly mis-remember what was discussed or what they agreed to at the time. So, yeah, sometimes I'm very reluctant to completely side with patients on the initial few encounters at least until I get other records.

I was fortunate enough to ride my situation out; if I needed to I could do next-month locums tenens but that's a short term solution to a long term problem. And credentialing for hospitals and insurances is a. fucking. bitch. Minimum 90 days but closer to 120-150 days to get paperwork in order.

I do think that it would be great if people don't have to subject themselves to more hazardous work environments for higher pay like factory positions. It'd be great if everyone could get a comfortable wage for even menial jobs, and there is clearly room in the budget for it given how inflated admin and C-suite salaries have gotten over the past decades.

I'm very confused. Do you think you that once you click the apply button or finish a phone call, you immediately know and are set up? The job hunt is a fucking slog my man. It took me 8 months to find a job in an area I wanted for decent compensation, and honestly most of that is because of the immense delay paperwork and administration causes. There is a LOT of downtime during the job hunt, even if you have your fingers in many pots. You can totally be a proficient job hunter and getter while still shitting around on reddit.

After about 8 months of searching, I finally found a job that I like. The only problem is that credentialing takes about 120 days and I have nothing to do in the meantime. I finished my training back in June, and instead of searching for work then I was more focused on a relationship that ultimately fell through. I've not been doing much since finishing, and I'm trying to do some traveling work; but holy crap everything either also takes forever to credential or is on the other end of the country and may also take 90 days to credential. This is infuriating.

And only then do they say eff the vitamin C and look for actual treatment.

Lady, I hate to break it to you, but you work a low skill job at bagel shop. Kudos to you for standing up for what is right, but someone could be trained up to do what you're doing in a day.

That's a really simplistic way to look at it. It could take quite some time to get back off the ground if the entire staff quits, and I'd imagine that something simple like a bagel shop might not even be able to recover from a month of no employees.

You're extremely young. Unless there's a family history of breast cancer I wouldn't try to worry too much, but that's an in-person conversation with your physician. Breast tissue in women <35 years is so dense that you probably wouldn't even get set up with a mammogram. You'd need an ultrasound, which has an even higher false positive rate.

You need to weigh your options, and there's a lot of nuance in this discussion. Also, make sure that your PCP is a physician (licensed MD/DO) and not a mid-level like a nurse practitioner or physician assistant.

How old are you?

Over-testing is something I would caution you about, as I have this discussion with a lot of my younger patients. Screening tests function on four principles: specificity, sensitivity, positive and negative predictive values. All of those are lower in younger patients specifically for breast issues. Much higher chances of false positives, and a positive screen needs to be followed up with diagnostic imaging. That builds up a lot of anxiety in low-probability patients, and it could lead to a negative biopsy which has the potential to deform the breast. I would have a follow-up conversation with your PCP.

I guess my take, which I would think is a reasonable one, is that we have now hit the point on the graph where anti-vaccination sentiment has intersected with anti-vaccine mandate sentiment. In an academic, theoretical sense they're very distinct entities. Functionally, pragmatically, however else you want to phrase it, they're now the same thing.

If you understand the science and the true benefit of the vaccine, and if you understand the overwhelming benefit (and it's honestly a disservice to use such a mild word like benefit because it's so much more than that) of public health, then you're cutting off your nose to spite your face to support a slippery-slope and fearmongering stance on individual freedom.

There are indeed case reports of fatal air emboli happening from cunnilingus, particularly in pregnant women.

From Williams Obstetrics 25th editiion, Chapter 9 "Prenatal Care: Common Concerns - Coitus":

"Oral-vaginal intercourse is occasionally hazardous. Aronson and Nelson (1967) described a fatal air embolism late in pregnancy as a result of air blown into the vagina during cunnilingus. Other near-fatal cases have been described (Bernhardt, 1988)."

What? What are you trying to accomplish? Are you keeping up with new research papers that are coming out? The entire foundation of scientific progress (and particularly medical progress) is that the field is dynamic and constantly evolving. There are things we've done in medical practice in completely unrelated fields decades ago, thinking they were beneficial, that took years of ongoing research and cases for us to understand that they were not the best practice; and the science changed.

I still believe the only good sex trafficker is a dead sex trafficker. But it's unbelievably shitty to murder someone and then label them with the most heinous crime you can think of to get away with it. For buying into that lie, I admit my culpability in it and I apologize for going as far as I did in defence of the dad.

That's really mature of you, kudos.
