Honestly, haven't had too much difficulty, other than the time I tried to push through the Hallowbrook clowns without waiting for my health to recover. In fact, Andrei didn't even get a single hit on me, now that I have max Celerity and that kickass uzi. Let's see...what else...
I did the Hengeyokai mission this time after not really paying attention to it in the first playthrough. I just straight up gunned down all the Giovannis I could find, and learned you can use some morgue slab to enter the zombie-filled basement (last time I had enough lockpicking to get down there manually). https://gamefaqs.gamespot.com/a/user_image/6/3/3/AABhrNAAEyy5.jpg Also found out that Ash Rivers gets captured by the Society of Leopold and branded, tortured, etc. I looked all around the cave for his key, but it didn't spawn where it should've, so I left his stupid ass down in the jail cell 'til the explosions locked him away for eternity. Yeah, that one wasn't too shabby, especially when you get upstairs and the spectral fire and smoke come out of nowhere. There were some other problems -- mostly getting stuck on scenery and weird platforming stuff -- but overall, I'd say it was a success. Maybe the unofficial patch corrects some of those issues.

You can talk to some people and they think you're dressed up, disfigured, etc. Kinda funny, actually. It's certainly one way to get used to cities' sewer systems, since you have to fast travel from the world map down there instead of taking taxis.
