I never beat the story mode of Brawl.

Ganondorf was the lackey of Master Hand. The plot was that Master Hand was turning all of the characters into statues and collecting them. It's later revealed that Master Hand is being forcibly controlled by an angelic creature called Tabuu.
IIRC, Tabuu turns all of them into statues, Ganondorf included. So insofar as he died, it was Tabuu that did it. When I try to remember what happened in Ultimate adventure mode I just remember Brawl instead.
