How has Adam Smasher not come down with Cyberpsychosis? David mentioned that he went full borg which I take to mean he is fully chromed out meaning highest risk of psychosis. And they said he's a legend which I took to mean he's been around for a while.

I could be wrong on both accounts tho so just lemme know lol

He is a cyberpsycho, just a high-functioning one. Smasher was a violent psychopath before he ever got borged up. Being a tool for Arasaka just gives him an opportunity to go on killing sprees without consequence and get paid for it."Humanity" is the stat in the tabletop game that governs how much empathy your character has. The more cybernetics you get/the more your mental well-being declines the score lowers until you hit zero where you become a cyberpsycho and your character is controlled by the GM. Mike pondsmith, the creator of the tabletop game, has said smasher started with humanity score of zero
