Ya, you had to actually git gud to see the true ending...

although sometimes it's dumb, like in the case of Ghosts 'n' Goblins where it's just "do it again"

I think 8 Eyes does it well where beating the game gives you the password for the next difficulty and is entirely optional (and FINALSTAGE in that game is hella legit)

Not sure how widespread that was, but the last time I think I saw it was with Contra 4 back in 2007.

In the older days, you didn't have a choice of difficulty, or it tended to be rare.
