you throw one through 9 innings but your team doesn't score for you so you go into extra innings with that ghost runner on base. Then your team scores during the extra inning and you pitch in the 10th without allowing any other base runner (besides the ghost runner)

Pretty sure it would still count, as the ghost runner did not reach base as a batter.

my question is, what if the ghost runner manages to advance to third, say on a sac fly or routine ground ball out?

yes.

the "Ghost runner" (not a ghost runner!) is not charged as a hit, walk, or earned bases.

It's be crazy if that's true and the runner scores. idk if that's a perfect game

A weirder question is what if you strike out the first batter and then second batter you get a double play on both the first base and third base.

Is that like a better than perfect game?

Yes. Pitchers have been hit before as well. It's super rare though. I've also seen bad hits at base make the ball pop up just enough to whack the catcher in the face. There's a guard there but I can't imagine it doesn't hurt from the sheer torque on your neck.

You're trained from a very very young age as soon as you let go of the ball to get your glove up and ready every single pitch.

It does happen sometimes. I remember an incident a few years ago where a pitcher got hit in the head and had to be rushed to the hospital for emergency brain surgery. Usually when a pitcher gets hit, it's a ground ball off the legs.

After seeing yadier molina take a wild pitch to the nuts I'd say it's scary being a catcher. Yeah he had a cup but was out for several games because of it.
