What does it change? I played it on normal with a friend for a few hundred hours, we got very far and did a lot, but didn't think hard mode would be anything additional besides harder enemies.

Hard mode in Terraria is more like "Part 2" of the game rather than just flipping a difficulty setting up (which does also exist).

After you beat a certain boss in the underworld, the entire world changes. Older bosses come back with advanced forms, and there are a ton of new bosses, events, and dungeons that unlock. Of course tons of new gear/items to go along with those as well

So is it like new game plus though, in regard that you can carry your player over and all equipment you have? Or do you start off fresh?

This legit might bring me back to this game, I was so disappointed when they cancelled the new game, but if there is all this content in hard mode that wasn't in the standard game, it's reason to play again.

Hard mode has been in the game for a while now, it starts automatically when you beat a certain boss. It's tied to the world. Not really a new game plus, just the next part of the game

No it is nothing like new game plus at all. Literally the moment that you kill that boss your world turns to "hard mode" and all the new things are added.

You should really try it. It sounds like you only played up to like Skeletron and liked it, if that is the case you will be blown away by what the game is now.

I wouldn't say hard mode is like part 2, I would say pre-hard mode is like the intro/preview to the game and then hard mode is when the actual game begins. Like, easy mode you kind of get to mess around with the different weapons and see what you like and then hard mode says yo, those are actual classes, now you can focus on a class and go crazy figuring out accessory builds for those classes.

The world you're playing in has some changes that happen to it once you go to hard mode and suddenly stronger monsters start appearing, you don't lose your character or stuff you built or anything like that, and certain things you do introduce new ores and everything. There's a pretty specific progression to the game.

And once you beat it, then you can check out tModLoader and play through mods like Thorium, that make the game totally wild.

i am so bad at that game, though i am stubborn and played on the highest available difficulty. i've never gotten all that far into hard mode, and once the update for master mode dropped i just kept getting wrecked. i was having fun getting wrecked, don't get me wrong, but eventually i just stopped playing. i'll get back to it someday. though i am a little bummed i bought it on gog and not steam, since they got some cool end of life features.

Basically everything changes.

New enemies, new bosses, new biomes, old biomes are now different.

The difference is like as big or even bigger than launch NMS to NMS today.
