Why would the machines choose to lose 800,000 batteries a year to imaginary heart disease

It's a dumb campy action flick. Dont overthink it, it's not deep.

The "aware" people die due to a psychosomatic response to psychological stimuli.

The "sleeping" people that die are liquified and used to feed the living batteries.

A self sustaining system of batteries. So to speak.

How do they explain blood coming out of people's mouths when they're beat up in the matrix

How do you know heart disease isn't something made up by the system to give excuse to remove someone for whatever different real life reason.

In terms of the story? The mind rejects unstable versions of the Matrix, sending people into shock & cardiac arrest. That being said, there's literally no reason Neo would be bleeding from the mouth the same as his injury from the jump. It's just dramatic or plot devices, like a cartoon. They were very much trying to be like Ghost In The Shell & the like. Going for an anime vibe. Sacrificing reason & rationale for impact & intrigue. Super meta, because it somehow works out, and translated gloriously into that kiss of life at the end of the film. If it gave them chills, they kept it in. Period

The easiest answer is that the machines had a symbiotic relationship with the humans plugged in due to the pact made with the world governments. If we die, you die. Kinda thing
