This is the case with the gangsters and the murdered doctor pulling a noodle stand.

The whole incident happened because the defendant six months prior to the case got shot and had a bullet lodged in his heart that the doctor failed to operate on. The doctor filed this info away in his safe and created a false report stating that the bullet was no longer a problem.

Just prior to the case starting, the defendant and his family go to this doctor for their first ever checkup and the doctor sends him a report stating that the bullet is still next to his heart, ready to kill him at any moment. Why would the doctor do this? He already hid the real report six months ago, why would he bring up the bullet in this most recent checkup?

The impression I got was that he knew the family had money since they were a major mafia family. He could keep milking them for medical procedures, check-ups, operations, etc.
