Just tell them you have an emergency and you need to leave.

Act like it's something undisclosed you've already discussed, and your boss will probably just go with it out of fear of embarrassment/awkwardness that they may have forgotten something.
At a previous job I completely forgot I was supposed to be taking my then girlfriend to an appointment that afternoon. I started my shift by saying to my boss "Remember I need to leave early today?" and he just nodded and said he remembered.
