I understand that the mediums are not 1:1. You are never going to get a perfect book to movie ratio unless either the book is written for that purpose (which would make it more of a screenplay, really) or if the movie captures every detail, making it like 9 hours long. Neither are practical and compromises must be made.

It doesn't bother me if it's done right, like in The Watchmen.
But sometimes it does when it's wrong, like in The Watchmen.

It's pretty much case by case.

If the book mentions in passing that a character has a mustache, don't get mad that he doesn't have one in the movie. If characters repeatedly reference the mustache, then you have a right to be upset about a cleanshaven actor stepping into the role. They're leaving out the character's signature trait.

Some changes are always going to take place. They need to streamline the story, they need inclusive casting, they need to cut the number of characters from 12 to 9. It's going to happen. The book can't be placed on a pedestal for which every inconsistency is an act of blasphemy. But some changes are more disruptive than others. If you turn a good guy into a villain, completely invert a character's personality or motivation, throw away good ideas because you're the great artist and you know better than the author, etc., that's the kinda stuff which warrants raised eyebrows. Even then, there are exceptions for which it works to the story's betterment.

Most of the time yes. Ready Player One was the last major offender for me. Completely strip mined the heart and soul out of the book to make a dumbed down bargain bin movie.

The Dark Tower film adaptation was also absolutely criminal.

However, Blade Runner was better than the book.

Hair color is a minor annoyance considering hair dye isnt that expensive.

One thing movies need to incorporate that books have is hearing the main characters thoughts. Not like constantly but when its something important but realistically they wouldnt speak it out loud like they do in movies.

I know some movies have but its not common

Only if it's a change I dislike and usually one that will cascade to create much bigger differences later. Obviously Game of Thrones got the worst of it as it went on.
