It's more because people are promoted based on their talents at their current position, not their talents at their future position; ending in a morbid equilibrium when people are finally promoted into positions they can't handle.

A decent human being would look at the position, be realistic with themselves and say Its probably not for me. Unfortunately most human beings are greedy, see that pay raise and dive into it without checking for water first.

The people at the top don't always see the issues that may be happening at the bottom of their own companies. Productivity reports are the worst.

On that note, friends and I were recently discussing this. One friend and his company has it setup where management positions are not the next step for higher pay. Engineers can work toward senior engineer positions with better pay that exceeds that of the managers. Their mindset is they want managers to be people who wants to be managers and not just for the pay raise.

Another friend and his company has it setup where becoming manager is a must if one wants more pay than the top engineer positions. I know of some engineering friends who wants to continue working as engineers but unfortunately for them, a pay raise requires almost giving up that aspect of their work.

That is a major factor. I think there is a name for this phenomenon. People will often keep getting promoted until they get to a level where they don't perform well, and often instead of going back down a level to where they excelled they are just stuck at the level they are mediocre at.
