I want a game where it has extreme/lasting consequences and people treat you differently for it going forward.

Or a game where picking the nice/"correct" choice ends up being the wrong thing, deconstructing ideal heroism and whatnot eventually.

Most often it feels like a budget thing. Maybe they ask you if you're ready before every mission, so rather than make this one an exception, they found it easier to just loop back around. Or maybe they were going to flesh out the refuse option and ran out of money or something.

I've seen this a few times. In KotOR, for example, you have to bribe somebody to testify falsely in court, complete with earning dark side points, in order for the trial to have its best resolution.

Then there are a couple of times when the answer that's framed as correct is politically-loaded, which ugh.

In the original Golden Sun if you choose no to accepting the quest to retrieve the elemental stars, you'll walk out of the sanctum and get a bad ending.

The Persona games often let you actually take an evil route, sometimes without you directly knowing. Persona 4's was particularly interesting, because it dramatically changes the ending narrative.

In the first Mass Effect, there's a hostage standoff that only ends with everyone alive if you choose the Renegade option. Although, to be fair, Renegade isn't necessarily evil. It's more sort of a chaotic neutral.

If that's the one I'm thinking of, it was so dang awkward how this guy--I forget it's been forever--but he either just died or he's standing there with a gun pointed at this woman and she's just like "you're a great scientist, daughter" like they were out at lunch partaking in a pep talk.

I think da problem is dev writershave a story in their heads, so tip make branching story choices kinda upsets their vision prob.

Some games have an alignment system though like Ogre Battle.

If you haven't already, try out Dragon Age Origins. It doesn't have any extreme/lasting consequences, but not all the main quest have a obvious good ending and one of them makes it so choosing the obvious good ending flips the switch in the prologue making you ask your self what did I just do, then to top it off, the obvious bad choice has a good prologue ending.

For what it's worth, Far Cry 4 has perhaps the most clever "choice ending"

Either of the two options turn out to be absolute garbage, albeit for entirely different reasons. But the hidden third choice is to simply hear out the "bad guy" before the game even begins, only to find out that he's probably the least of the three evils.

Golden Sun, Suikoden II, and Breath of Fire 2 are three games I can think of that you can force an early game over/bad end by making such a choice when it is seemingly innocuous.

In Golden Sun, you can deny being sent on the journey to begin with which will trigger it, in Suikoden II you can decide that what you've been through has been too much on you and trigger an alternate ending 70% of the way through the game, and in Breath of Fire II it's really irritating because they ask you to go on after a very aggravating boss fight and make it seem like a point of no return and ask you if you want to take a breather first and choosing it triggers the bad end and you have to fight the boss over.
