To be honest I haven't read the books, but the movie left two options as I remember it:

1) He didn't actually do all that shit and he was just suffering from delusions and hallucinations caused by his psychosis, or
2) He really did kill all those people, but the real estate agent covered it up so that it wouldn't affect the selling price of the property

I like option 2 more. I love the idea that America let him get away with being a serial killer to protect the bottom line, but I suppose 1 is more likely. I mean he went on a legit GTA killing spree in the final 20 minutes.

Its intentionally left vague even in the book. The point of the story itself is that what hes doing isnt an exit from his mundane unfulfilling grind of a life, but whether its a flight of fancy or hes actually killing people isnt explained.

Certain parts seem obviously false, like in the book he literally has like a shootout car chase with the cops and the stuff with the ATM asking for a cat obviously isnt going on. But other parts seem plausible or possible

Probably a mix of both or that the killings happened, but not exactly in the way shown. The chainsaw scene seems that way. No way people didn't hear any of that. Patrick probably did kill the prostitute, but not how that happened. He was watching Texas Chainsaw Massacre earlier and probably mixed in that in his delusions of killing people.

Same for the wild shootout later. He probably did kill that old lady, but didn't get in a shootout with the cops. When the cop car explodes Patrick looks at his gun with a quizzical look on his face like "Wait, it did that?". A lot of that must have just been in his head.

So I think it's more option 2, but Patrick is an unreliable narrator which is shown more in the book. He killed people, but there is a lot of fantasy still involved.
