Nintendo was pretty strict back then, so you had things like each castlevania game having topless statues altered, medusa altered, the cross edited to look less like a cross, a level having green goo instead of red blood in super castlevania 4, etc. etc. When it comes to more modern games, I guess tales of berseria could count....it had a short scene at the beginning of the game altered.
