Why don't the obese on their third 100,000 dollar bypass get lectured and treated like garbage? Why isn't the person with a fractured leg from a very dangerous hobby left to suffer for hours on a gurney with no pillow in a room with a camera?

Why aren't people with stints and heart transplants from decades of eating badly eventually told not to come back until they fix themselves? Why do heroin users get methadone clinics?

Why do people with an STD get a chuckle and joke from the MD about wrapping it up and we get treated like we personally offended the doctor? Why is it easier for a smoker to get a lung transplant than to get a liver transplant?

Half the people in the hospital are there because of bad choices. But alcoholics are the bad people who get 2 or 3 chances then it's out on the street, not even a handful of librium.

Why in 2019 does everyone just say to alcoholics go sit in a room with bad coffee and heavy smokers and tell drinking stories and cry for an hour.

I mean what the shit people.

We don't treat alcoholics any differently than anyone else with substance abuse problems. It's a mental illness and should be respected and treated as such.

If you're talking about people going into EtOH withdrawals, that has more to do with the seriousness of delirium tremens.

Its an issue that goes with homelessness/being destitute in this case. Theyre one of a few groups that are easy to identify within the hospital system as probably not being able to pay any bills. So theyre rushed out of hospitals ASAP so they dont hog bed space.

There are quite a few good oral medications for Type II diabetes that are affordable.

If you're prediabetic, consider starting metformin. It's been shown to prevent the progression to Type II DM.

Unfortunately, insulin is the only treatment for Type I.

Consider starting glyburide or glipizide next if you find your A1C is still having trouble.

They're both $4 iirc.

One thing, though, is that metformin typically doesn't cause hypoglycemia but many of the other oral meds can, so they should really only be taken if you have DM2

That is actually not true.

Here is a list of contraindications for a lung transplant. Active smoking is one of the criteria.

Librium (and ativan for that matter) is not used outpatient because withdrawal usually resolves inpatient therefore you don't take it long term.

Also, it's not like we "don't care". We provide a lot of social placements/assistance to help treat alcohol intoxication/abuse but the patients don't care/lost to follow up/goes back to drinking again. I can't help people who don't want to help themselves.

Because surgery yields a poor benefit to risk ratio if the main problem is mainly diet. Methadone can be given orally with no risk of surgery complications and is given in a monitored setting. Two different things.

In my hospital supposedly, patients with no insurance gets emergency medicaid only for their stay. But the costs do add up. It is true that in my hospital, we do tend to rush out patients who are stable for discharge (social workers and the higher ups are always on our asses to do so), but we definitely make sure that the patient is "cleared" for discharge when the acute problem is corrected.

Not a doctor, but I'm pretty sure different medicines don't work the same for every one. It's great that there's a cheap insulin, but I doubt it's the one every diabetic needs.

Alcoholics brought their problem onto themselves much like the obese, but they are also a major burden for liver transplants which are desperately needed for others who need a new liver for no fault of their own, cause relationship problems for everyone around them with their shitty behaviors and attitudes (and don't get me started on the abuse issues many of them have), can never hold down a steady job because they frequently get disciplined or fired for showing up to work drunk and/or caught drinking on the job.

That is the way they did my Klonopin. 8 years of being on it and then they yank you off cold turkey because the DEA and/or medical board threatens to start shutting docs down if they keep on writing it.
