Outside anime/manga, what are some other examples of this? I dont mean a new book coming out after the main series is finished. Just not finished at all.

tbf, when they first began the show, they likely believed that GRRM would have finished the books long before they ran out of source material

Just get competent writers. GRRM isn't the only competent writer. Lots of shows change writers or showrunners during their run and are still good. Not having source material to draw from doesn't excuse shitty writing.
