Say a man has split personality disorder and the second identifies as female. Second personality takes over for a long while, say a few years. During those few years she requests hormone therapy and sex reassignment surgery. What happens?

But what if Male personality comes back and objects to no longer having a penis? Shouldn't his rights be considered too?

If I'm not mistaken, it's not like you can just walk into a surgery and say "hey change my sex plz", you've got to go through a significant time living as the opposite gender pre-op first and stuff.

It'd be very likely that, during that, they'd uncover the split personality issue, and probably have some plan in place to handle such situations.
