You can go from any gear into any other gear.

I would advise not going into or out of reverse if you're still moving, though.

As long as you're at a suitable speed for the gear you're switching into, it's fine. I almost always skip 4th when I'm not specifically intending to end up in it.

Likewise, you can start in a higher gear than 1st too, although it's generally not a good idea (too much strain) to start in anything higher than 2nd.

Have no idea but I think it's one of those things where it can serve a purpose but it's not a good thing.
