...world map that is used as an in game item (you have to take it out and look at it), a game world with realistic scale like instead of setting it in a whole country and having cities being tiny, having it all set in a small region with realistic sized communities and populations, and a decent combat system.

Literally my fantasy video game. I'd make a game just like this if I had the skill/resources.

Also realistic pee/poop needs and you need to wipe your ass after pooping.

Since you're probably gonna be in the middle of the forest you're gonna have to squat and rub your ass on the grass instead. And the graphics will be realistic enough to leave a little brown train on the grass that only goes away after it rains.

In this fantasy of mine, the quests are as such that they work for area they're in (meaning they're located around the community or quest-giver's location), make sense for your character to pursue (doesn't make sense for a hero or wandering adventurer to be picking up x of a certain random item or delivering a letter to someone's sister in a city across the map), or are big events that would make sense to relocate for.

I would go with Morrowind, with mods. Necessities of Morrowind definitely gives the "realistic needs" that you requested, I'm sure that with a little digging that you could meet the rest of your requirements through mods.

'Realistic' is sort of shitty for games, though. Kingdom Come Deliverance promised the same thing but didn't really deliver on it. There are a lot of things that we assume are 'realistic' and aren't. For example, archery. Traditional war bows actually require a great amount of strength. So much so, in fact, that when they exhumed medieval archer's skeletons, they found that the bones in one arm were actually warped from being overdeveloped to the point where it would have been identifiable just looking at one.

Then, on top of that, people assume that you can survive several arrows when in reality, even a single arrow to a non-vital limb (such as your leg) can and often were, lethal. Even if you survive, you'd likely have to amputate the limb or suffer from lifelong injuries because of it.

So yeah, in a 'realistic game' bows would be so ridiculously overpowered.

what's funny is that in games archers are usually female
not that women are weak, but upper-body strength usually isn't their forte
