Somewhere near the beginning of each year, your employer(s) of the last year give you the form (its blue and white) and it shows how much money you made over the last year working for them, and the taxes that were withheld from your pay.

You use it to prepare your tax return and it's often used as proof of income and employment if you're doing something like applying for financial aid

If you're self employed, work 'under the table', or have never had a job, you won't have received one.
