I always hated this one.

Is it no sauce?

Is it sauce but no toppings?

Is it nothing at all but bun and meat?

Is it simply saying "I don't want anything extra, just give it to me how it normally comes"?

Just say what you specifically want instead of forcing people to interpret a vague single word descriptor.

It means no condiments except cheese. So bun, meat and cheese.

That's pretty much just nitrogen, with a bit of oxygen and argon, and trace amounts of other gasses.

Where I worked, plain meant just the meat and bun, with cheese if they want a cheeseburger. So if they said plain that's what we would do and if the customer got angry, well...we tell them that's what plain is for us and he'd have to clarify next time.
