Submit to Gehrman, refuse him, or refuse him after consuming 3 cords. --- The ball is round, the game lasts 90 minutes. That's fact. The whole world of Bloodborne is a dream world maintained by the Moon Presence, which is the monster you face as the final boss. The point of the dream is to stop the other Nightmares from taking control. Plus, as mentioned there are three endings. You ONLY fight the Moon Presence in the ending where you consume the umbilical cord, otherwise the game ends after the fight with Gherman.
