a lot of them werent very balanced which i think is where most of the video game difficulty comes from
good example are those beat em up games in arcades, like X-Men, The Simpsons, etc. there's very little strategy in them and not much you can do to avoid death like 90% of the time. the game is really just designed around you feeding quarters into the machine constantly after you die --- A Green Butter Alt(TM) http://i.imgur.com/LhwwG.gif ... As I work my way through the games on the NES classic I completely agree. The ability to save at any time, basically unlimited lives, makes things a hell of a lot easier.

Wolfenstein 3D had a lives system, but its usefulness was rather moot. Whenever you die, not only do you have restart back at the beginning of that level, but your inventory will be reset to a knife and a pistol with 8 rounds. In other words, avoid death at all costs. You could save anywhere, but still. Lives are virtually useless.

Arcade games are designed around playing for score, feeding a quarter zeroes out your score.
