The majority of the mistakes are mistakes made by humans in the heat of the moment in high stress situations. Not by some guy sitting behind a desk checking the wrong box on a form. As long as people are put in high risk high stress situations mistakes will ALWAYS be made. I'm not saying it's not wrong that people's lives are ruined but there is no way too 100% neutralize the possibility

No, I just avoid interacting with them whenever I can, and if forced to for whatever reason, I keep those encounters as brief as possible so I don't end up hurt/dead.
