Two inmates escape an insane asylum. The second inmate is too scared to jump across the gaps between the houses, because it's too dark. The first inmate offers to shine a light so he can walk across. The betrayal of expectations starts here, when he asks, "Do you think I'm crazy?", because you're expecting them to know that you can't walk across beams of light. Instead, he's told that he'll just turn the flashlight off halfway across. The entire joke builds upon itself.

What Alucard said. It's also a reference to the two of them (as Joker himself says. The situation they're in reminds him of a joke). Batman is trying to offer Joker a way out of the craziness or at least out of the huge cycle of destruction they're both in. Batman can make it out easily since he's normal enough, he's the first inmate. Joker is the second one that's too afraid or too damaged to make the move to escape. He also can't trust Batman to truly help him out so he has to decline the offer.
