Well a lot of the time these attacks are terrorism. Also the intent of terrorism is to drive up paranoia and irrationality, so if it's doing that, it's obviously extremely effective.

If you're talking less about domestic attacks and more about problems in the Middle East which often aren't technically terrorism (though often are), it's because the social-political-religious issues that have led to conflict with a large number of factions are extremely complex and wide spread and not often well understood by those who aren't experts in the field, so in essence it is a buzzword to label Middle Eastern factions who aren't fans of the West.

One to even a few co-ordinated (but isolated) maniacs aren't really terrorists unless it's something on a huge scale, like the OKC bombing. ISIS, Chechen rebels, the IRA, etc., are/were legitimate terrorist groups. It's kind of a gray area, but I agree.
