When Aang refused to let go of Katara I thought it left him unable to open the last link. But in the last fight all it takes is a rock poking it to completely bypass the whole Katara thing?

I can't say they meant it like this but I think a part of that was Aang's struggle to not kill the firelord but with him losing the fight and his friends being in danger maybe he decided he needed that power without thinking?

It wasn't permanent, he was entering the avatar state in the same episode right before Azula hit him with Lightning in the last episode of season 2.

His chakra was probably blocked by the lightning he took at the end of Book 2, and more physical trauma undid the blockage. Remember that Katara was trying to unblock it at the beginning of Book 3 with healing, but she stopped because it was hurting him.
