at least compared to retail, service, community/government, labor, trade, etc. jobs I've had --

is that everything is a tug of war for liability. Someone is always trying to divert blame elsewhere, or find something to snitch on to make themselves look better/more competent at the expense of someone else's mistake.

I get that catching mistakes is important to avoid patient harm - but literally it can be the most minuscule mistake, and people are rewarded for catching it. Usually when you receive some sort of accolades it's from finding something someone else did wrong -- it's never something like, "wow, you really made me feel comfortable about ___ and helped me cope with the situation," or "wow, you really went out of your way to make sure I'm informed," or "wow, you really give excellent service!" In retail if you'd receive accolades it was usually for one of those reasons, like someone would tell corporate that you really helped out by special ordering x item or knowing a lot about products, you helped someone locate something that wasn't at that particular store by letting them know where else to go or contacting other stores, just being friendly, etc.

In retail it was about what YOU did, not about what someone else DIDN'T do.
I know you could say that's just the nature of capitalism and blah blah "dog eat dog," but I literally never felt this mentality at any retail or service job, or any contracting company I've worked for, at any factory or warehouse, at WIC, etc.

I recently received some congrats for catching a mistake someone made, and it feels cheap getting thanks at the expense of someone else's mistake. Catching a mistake should just be your job, not an accomplishment -- and someone's mistake shouldn't be brought to light anymore than it needs to be for liability purposes.
