I mean.... it's a nice expression but it's not meant to be a "universal truth" it's one of those sayings you use in response to someone that goes out of their way to assign nefarious intent to things.

Traditionally this saying is used to reign in wild speculation and to suggest you take a step back and consider other possibilities before assuming someone has it in for you or bad intent.

Not never. Generally, and give anyone you don't know better the benefit of the doubt. As you get to know someone however, it can get to the point where you should attribute it to malice.
