It seems reasonable to state that if puberty blockers can help trans persons better realize living as their gender then living without it may carry lasting effects from having gone through puberty as the other gender.

Why not either create 4 leagues:

Cis women
Cis men
Trans women
Trans men

Or... just let everyone compete without separation. Boom, solves the problem.

Well, that's the only solution I see if the main complaint is "fairness." Wouldn't be fair to say someone can't compete at all due to gender identity.

Hormone therapy does not change all elements of male physiology such as bone structure, lung volume or heart size.
