Bad Breath. Nothing else can compare to it. The worst version of it is in Final Fantasy X where Marlboros are guaranteed ambush encounters and use it immediately. Then you watch as your party kills itself in seconds.
This specific one. There's an area absolutely crawling with these bad boys too.

Just a heads up TC, since I know you havent played FFIV yet... a vote for Bog Witch (one of my nominations) and Bog Toad (gunplagirls) should probably count towards the same nomination. They come in a pair. The witch tells them to croak, and each one casts toad. So its sort of a tandem attack.

(They were called ToadLady and TinyToad in the original translation.)

Yeah any move that cause multiple status ailments are generally just bad news in these games and Bad Breath is the poster child for that. So that also gets my vote

Would like to give an HM to a move I believe was called X-Zone in FF6. One time I was grinding a tower in the WoR (I forget its name) on the upper levels to get growth points for my characters to learn skills. I must have been dong this for about an hour or 2 and finished up and made my way to the exit. Only for an enemy on one of the lower floors to use that move and OHKO my entire party. Game Over, made me very unhappy (fyi there was no save points in that tower).

I was going to post "Anyone who says Aire Tam Storm needs a lesson", but you already posted it so here's your lesson!

Does 1111 per Materia equipped. Emerald is a joke. You can beat it with a single character, some elixiers/x-potions, a few Counters, Mime and a Level 4 Limit break. There is nothing bad about this attack if prepared. In fact, it's Stomp attack and return fire eye laser things hit harder than Aire Tam Storm.

As for my nominations:

Various - Malboro's Bad Breath
FF7 - Midgar Zolom's Beta. Mostly because I spend like two hours here trying to learn it and kill it when you first encounter it.
FF7 - Super Nova
Thanks! I do know how it works though. Im approaching the definition of the topic as attacks that really stand out in my mind from experience as being really detrimental, difficult to overcome, or otherwise problematic. A-T Storm was exactly that for me a couple decades ago. I didnt know the algorithms behind it and common sense was to deck out your party for the battle at hand. Obviously in the age of the internet, easy access to info and people sharing strategies changes things but thats now, not then.

1) Stony Glare (Chac, Final Fantasy X-2). It's possible to get status protection against the worst Bad Breath has to offer, even in FF X. No such luck with Stony Glare, which flips a giant middle finger at immunities. And given Chac's lightning-fast speed, she will probably shatter the afflicted character (permanently removing them from the fight) before you've had a chance to cure the petrification.
This. As if her fight in FFX wasn't agonizing enough, now she's back with absurd speed and attack power and this shit that just blatantly ignores complete petrification immunity for no reason.

Honestly, this. Bad breath can sometimes result in a game over if it's nasty, but fleeing is an option, so is ribbon (sometimes) or remedy/esuna.

But there is no fleeing or undoing a shattered sword. It's reset or accept -- screw that nonsense. Fuck you, Meliadoul, notably, and all your ilk like the knights outside Lionel castle.

FF14 - Final Sting (Gotta agree with this one, it's the Sprout Trap, because they don't usually focus-fire them unless told).
FF10 - Banish - Nothing like centering your game around a summoning mechanic then giving boss(eS) the ability to circumvent using them for more than 1-2 turns

Bad Breath sucks ass. Stupid Malboros.
The Popo posted...
Malboro (any game), Bad Breath - If you dont have the proper gear on, itll wipe out your party instantly

Sorcerer/Mindflayer (FF1), Regular Attack - This fucker has a random death ability attached to his regular attack, so encountering them in the ice cave is incredibly dangerous. Especially in a game where the only way you can revive someone outside of towns at this point is if you have a white mage that knows Life, and you can probably only cast it once or twice at this point. I hate these guys...

Bog Witch (FF4), having her toads cast Toad - This is just an annoying enemy. She fights you with 6 toads accompanying her, tells them to cast Toad, and then you gotta wait for all of her damn toads to do their thing. It takes upwards of a minute, youll have random party members as toads now, and then shell repeat the process. It isnt a difficult fight... hell, I dont know if you can actually lose to them. I dont think they attack. They just waste your time.

These are all solid choices.

Bad Breath (FF8/10 Malboros)
Super Nova (FF7 Safer Sephiroth)

Bad Breath isn't too bad in most games, but Malboros in 8 and Great Malboros in 10 are obnoxious bullshit machines where if you don't have immunity to several status effects you just auto-lose the moment the fight begins. Great Malboros always attack first unless you use a specific weapon ability and always use Bad Breath on their first turn unless you use Provoke. FF8 Malboros will always use Bad Breath right away and you can't realistically run from them before they can act, nor can you kill them in time without luck on getting limit breaks because they have gigantic HP. Super Nova, meanwhile, is just a tedious waste of time to sit through.

Honourable mention to practically every full party attack by random enemies in the DS version of FF4. Around the halfway point of the game you reach a point where almost every dungeon has a potential random encounter with up to 5 enemies who 2HKO most of your characters with a group attack, always attack before all but one of your characters who cannot kill even one of them with a single hit, and always get to act before you can run. If the AI spams their AoE moves you just lose.

Maybe if you learned to use mechanics (curse rings, aka the last half of the game on ez mode for 90% of fights) then you should never wipe to regular enemies. :l Or, you know, using gear to increase your speed so you don't get ambushed as often.

Ambushes are irrelevant, enemies move first regardless (from my experience even Edge was pretty random on acting before enemies, if there was a group of five he'd often act after two of them, and he's the fastest dude, so I would not have expected speed boosting equipment to help. Maybe it does but the game communicated pretty clearly to me that speed was not very important by showing me my highest speed character still only sometimes going first). Every single non-boss fight death (all five or six) I ever suffered was the exact same formula of group of enemies showing up and killing with AoE attacks before I got more than one turn. Maybe there's equipment to absorb some elements but there's no way to know of that in advance of entering an area and dying, and with how rarely the enemies act intelligently enough any counter measures taken in response to one death would be unlikely to ever affect anything. With how the game lets you trivially breeze through areas otherwise (abusing that MP restoring ability and the fact that almost every non-boss prior to the final dungeon is vulnerable to Stop you just full heal at will on helpless enemies whenever you want) it's really not going to ever feel worthwhile to go out of my way to deal with a very low RNG chance of death that requires me to waste far more time wandering off to get new equipment than I'm likely to spend on just chancing it.

Oh right, speaking of FF4 DS I remembered an actual third nomination

Reverse Gas (Lugaeborg, FF4 DS)

In theory it's a cool gimmick, he periodically causes damage and healing to be reversed so you have to use healing abilities on him and hit your own team. In practice, it's a broken mess because he spams it faster than the delay on actions allows you to fight. It's entirely possible for you to input commands for every character and have Lugaeborg Reverse Gas before any of them come out and then do another one in between every single attack. At times I landed damage on the fucker because in the brief delay between inputting the Attack command and it coming out (which I believe is the shortest delay of anything in the game besides maybe items) he used it twice. I also failed to get an attack to hit because he Reverse Gas'd three straight times in between input and action. When I eventually got sick of his shit and decided to just nuke him with a couple of reversed Elixirs, it still took several attempts because even waiting for him to use a Reverse Gas before inputting the Elixir on him, he'd still do another one before the Elixir could come out a lot of the time. I don't know if this was supposed to be this janky or if the fight is glitched but either way it's a total gong show.

No I'm pretty sure going by what the game tells me is the game's problem. Keep in mind you're bitching at me for not knowing to use a random drop that doesn't even tell you what it does (a recurring problem in this game with some equipment). That would probably take as much time to get 5 of as it actually saves from the 5ish times it will matter. Pretty sure there's not much of a personal failing involved in not looking up a guide for where to grind and why.

Nominations are closed. I combined any ejection attacks into one category, since there were a lot of split votes among them. I did the same thing for any Frog/Frog Song/Toad attacks.

FFs Malboro: Bad Breath - 34
FF7 Safer Sephiroth: Supernova - 11
FFs Whirlsand/Snort/Goannai - 11
FFs Frog/Toad - 6
FF9 Necron: Grand Cross - 4

FFs Steal - 4
FF6 Magic Master: Ultima - 3
FF14 Final Sting - 3
FF7 Emerald Weapon: Aire Tam Storm - 3

We are left with 2 votes on the remaining choices as follows:

FF7 Carry Armor: Grab Arm - 2
FFs Master Tonberry: Everyone's Grudge - 2
FF9 Ozma/Hades: Curse - 2
FF1 Sorcerer/Mindflayer: Attack - 2
FF10-2 Chac: Stony Glare - 2
FFT Assassin: Stop Bracelet - 2
FFT Break equipment - 2

Since I get first revote, I choose "FFs Master Tonberry: Everyone's Grudge "

FFs Malboro: Bad Breath - 34
FF7 Safer Sephiroth: Supernova - 11
FFs Whirlsand/Snort/Goannai - 11
FFs Frog/Toad - 6
