They have deals with the hospital, the fee they charge the insurance company is 10x less than the bill you would get if they presented it to you without insurance.

Its essentially like this. Your doctor bills you for an amount. You pay up to your deductible and OOP. Once you hit your deductible your insurance will start covering a certain % for procedures. Lets say they cover 90% of your shots or whatever. You then pay 10% until you hit your OOP then your insurance should cover it all most of the time. There's a lot more to it but thats like the gist of it --- The night brims with defiled scum,and is permeated by their rotten stench. Just think.
