Like why do people make it so complex with the keto diet or intermittent fasting? Isnt it just basically calories in vs calories out?

Pretty much.

A "slow metabolism" doesn't create matter and energy out of thin air. Living and performing any sort of movement/work requires a absolute minimum amount of energy. Without intake greater then that usage you will lose weight no matter what even if you don't get out of bed the entire day.

If you consume 1000 calories a day and your basal rate is 1500....you will lose weight. It may not always be from where you want but you will lose weight. Whether its fat, muscle, or even organs...you will lose weight.

Keto is supposed to only burn fat where as just cutting calories will do the same, but also reduce muscle mass if not supplemented by lifting.

Or something, idk. I'm not fat.

Why do people gain weight when taking certain medicines? Im no dietician but Im pretty sure theres a hormonal component

Total weight, sure, but the type of tissue you lose is different. Just not eating, and being sedentary, is going to take a toll on your whole body. Also, different methods have different effects on your will. Keto is great for losing those snack urges.

Hormones also play a role. Its not as simple as calories in calories out for everybody, no matter how much people who havent had to deal with weight loss will swear it. Thyroid problems, adrenal problems, medications, etc. all affect your hormones and your ability not just to lose weight, but to lose mass in the right parts of your body.

Calories in, calories out. You can control your calories in, it's very hard to control your calories out. That's where types of diets, metabolism, workouts, et cetera come in.

Special diets are for people who struggle with simple portion control. But theyre also not particularly sustainable across multiple years, so you could look at them like gateways to thinking about food differently and losing life threatening weight.

I was on low carb, keto and a bunch of others for a long time. I lost weight but couldn't sustain it. So I decided to eat whatever the hell I wanted, with a calorie limit. I just try to go for high protein and 1200-1500 calories a day. I feel satisfied and the weights dropping. I feel like I can sustain just by calorie counting.

My one bad habit is drinking way too many diet sodas and not enough water.

because those things can make eating at a caloric deficit not suck so much. eating 1500 calories of keto food is a hell of a lot more satiating than eating 1500 of a more carb heavy diet, particularly if a lot of those carbs are sugar.

Trust me when I say its so much easier to be proactive and just not put on much weight than to try and lose weight. But yeah, the common denominators with getting in shape are simple enough - eating well in reasonable portion sizes and vigorously working out routinely throughout the week. The exercise is technically not nearly as big a deal as the diet but I think psychologically locking in a good workout routine is the game changer as far as making real lifestyle changes.

You can lose weight in very unhealthy ways. Muscle atrophy is no joke.

It's way more than just CICO. You have hormones (like insulin, ghrelin, leptin) that are out of balance, nutrients that are depleted (potassium, magnesium, zinc, vitamin A, vitamin D), and ratios of EAAs and EFAs that are off (more omega-3 fatty acids to counter the inflammatory effects of omega-6 fatty acids).

Your body craves nutrition, and inflammation is a result of poor nutrition. Weight gain is just a symptom of a body that is getting calories, but not nutrition. If you start eating foods that are lower in carbohydrates (these deplete nutrients the most while also triggering insulin and leptin the most) and richer in micronutrients (more nutrients allows cells to work more efficiently), your body will become more stable and productive, your inflammation goes down, and you lose weight as a result.

True. Weight loss itself is "simple"

Doing so in a healthy and sustainable way is less so.
